How AI in Healthcare Can Mislead Doctors with Biased Data

In the intricate landscape of healthcare, the role of artificial intelligence (AI) has been both a boon and a potential bane. While AI holds the promise of aiding doctors in making more informed decisions, a recent study published in the Journal of the American Medical Association (JAMA) unveils a disconcerting reality. 

The study delves into the repercussions of AI in healthcare, specifically shedding light on the dangers posed by biased information provided to doctors. Despite rigorous efforts by regulatory bodies like the Food and Drug Administration (FDA) to ensure the reliability and safety of AI tools, the study suggests that even with explanations, doctors may be misled if the AI learns from flawed and biased data.

Exploring the impact of AI in healthcare diagnoses

The research, led by Sarah Jabbour, a computer science Ph.D. student at the University of Michigan, focused on AI models designed to assist in diagnosing patients with acute respiratory failure—a critical condition where patients grapple with breathing difficulties. Collaborating with medical professionals, including doctors, nurse practitioners, and physician assistants, the team investigated how AI’s decision-making impacted diagnostic accuracy. 

Surprisingly, when medical professionals relied solely on the AI’s decision, their accuracy increased by approximately 3%. But, introducing an explanation, represented as a map detailing the AI’s focus on a chest X-ray, further improved accuracy by 4.4%.

The study took a crucial turn by intentionally introducing bias into the AI’s decision-making process. For instance, the AI might inaccurately suggest that older patients, aged 80 and above, are more likely to have pneumonia. 

Jenna Wiens, a professor involved in the study, highlighted the inherent biases in AI when learning from flawed data, citing an example where gender-based misdiagnoses could lead to skewed results. Unfortunately, when doctors were presented with explanations from the biased AI, their accuracy plummeted by about 11.3%, even when the irrelevant focus of the AI was evident.

The challenge of unraveling AI’s biases

Sarah Jabbour acknowledges that while AI explanations offer a potential avenue for improvement, the study underscores the challenges. The drop in performance, consistent with other studies, signals that even with explanations, AI models may still mislead medical professionals.
