A $200 million attack on decentralized lending protocol Euler Finance in March was reportedly caused by a bug introduced as a result of a fix to a previous bug bounty report. Pseudonymous whitehat Kankodu claimed that their report on a “first deposit bug” had been awarded $50,000 and subsequently fixed, but inadvertently introduced a new function that enabled the larger exploit. The Euler team was able to recover most of the funds taken in the attack.
